• Prescribed Fee: A fee of ₹50 (plus applicable GST) must be paid at the post office to process the application (p. 1).
• KYC Documentation: You must attach self-attested copies of your identity and address proofs (such as Aadhaar or PAN card) (p. 1).
• Joint Accounts: If it is a joint account, signatures and KYC documents of all account holders are mandatory (p. 1). [2, 3]

• Account Specifics: The exact account number, scheme type, and the name of the Post Office/Branch Office where the account is held (p. 1).
• Personal Details: Full name, guardian/parent names, and both your original (at the time of opening) and present addresses (p. 1).
• Financial Details: The date and particulars of your last transaction alongside your last known credit balance (p. 1).
• Reason for Reissue: Detailed circumstances explaining the loss or damage of the book, and whether the issue was reported to the police (police report is optional) (p. 1).

• Verification: The Sub-Postmaster (SPM) or Postmaster will verify your inputs against official office records before approval (p. 1).
• Torn Passbooks: If you are applying because your passbook is simply torn or spoilt, you must physically surrender the damaged booklet upon receiving the duplicate (p. 1).
• Illiterate Depositors: If the applicant cannot sign, a thumb impression is required alongside the signature, name, and address of a witness (p. 1).
