SBI Form C – PPF withdrawal / Closure Form
Form-C is the official application used by subscribers to request withdrawals from their Public Provident Fund (PPF) Account under the 1968 scheme.
1. Withdrawal Eligibility and Details

The subscriber must provide their specific PPF account number and the exact sum they wish to withdraw in both figures and words.

The applicant must confirm that a period of seven years has expired from the end of the financial year in which the initial subscription was made to qualify for the withdrawal.

The physical PPF Pass Book must be enclosed with the application.
2. Withdrawals for Minors

If the withdrawal is being sought from a minor’s PPF account, the guardian must provide a certification.

The certification must state that the funds are required for the use of the specific minor, who is still alive and remains a minor.
3. Payment Options
Upon sanction, the subscriber or guardian can choose how to receive the funds:

Direct Credit: The amount can be credited directly to an associated SBI Savings Bank (SB) account.

Demand Draft/Bankers Cheque: The bank can issue a DD or BC in favor of a designated recipient.

A Revenue Stamp and signature (or thumb impression) are required on the receipt section of the form.
4. Internal Bank Verification
The bank utilizes an “Office Use Only” section to verify regulatory compliance before sanctioning the amount, checking:

The date of the initial subscription.

The total amount currently available in the PPF account.
-
The date on which the last withdrawal was permitted.

The maximum amount currently available for withdrawal under paragraphs 9(1) and 9(3) of the PPF scheme.
