The Sukanya Samriddhi Account (SSA) forms for Bank of India (BOI) are standardized under the Government Savings Promotion Rules. These forms are used to manage the savings scheme specifically designed for the benefit of a girl child.

Below are the key forms and their primary functions:

Account Lifecycle & Operation

SSA FORM Opening of Account (Form-1): The primary application to open the account. It requires the birth certificate of the girl child and the KYC documents (Aadhaar/PAN) of the guardian.

Form – 1, which is the official government application form used to open a Sukanya Samriddhi Account (SSA) at any Indian Post Office or authorized bank. [1]
The key points and requirements from the form are structured as follows:

Account Ownership & Eligibility

Target Beneficiary: The account type is strictly classified as Minor, opened exclusively for a resident Indian girl child.

  • Account Opener: Must be opened and operated by a Natural or Legal Guardian on behalf of the minor.
  • Operating Rules: The guardian operates the account until the girl child reaches majority (18 years old), after which she operates it herself.
  • Exclusivity Declaration: The guardian must sign a binding declaration stating they have not opened any other Sukanya Samriddhi Account for the same girl child anywhere in India.

Required Information & Fields

Depositor (Minor) Details: Full name, exact Date of Birth (both in numbers and words), and details from her Birth Certificate (Certificate Number, Date of Issue, and Issuing Authority).

  • Guardian Details: Full name, relationship type, Date of Birth, Aadhaar Number, and Permanent Account Number (PAN).
  • Contact Information: Present and permanent addresses, phone number, mobile number, and email ID.

Mandatory Attachments & Documentation

  • Photograph: A physical passport-sized photograph of the guardian/applicant must be pasted on the top right.
  • Birth Proof: Birth Certificate of the girl child.
  • KYC Identification & Address Proof: The form accepts 5 officially valid documents:
    1. Passport
    2. Driving License
    3. Voter’s ID Card
    4. NREGA Job Card (signed by a State Govt. officer)
    5. National Population Register (NPR) letter containing name and address
  • Specimen Signues: The guardian must provide three distinct specimen signatures on the form.

Nomination & Witness Rules

  • Nomination Facility: The guardian can nominate up to 4 individuals to receive the credit balance in the event of the guardian’s untimely death.
  • Minor Nominees: If a nominee is a minor, a separate adult must be appointed to receive the funds during that nominee’s minority.
  • Witness Requirement: The nomination section requires the signatures, names, and addresses of two witnesses.

Current Scheme Context (FY 2026-27)

While the form collects the application data, the underlying government rules specify that you can initiate the account with an initial cash, cheque, or demand draft deposit. The scheme currently offers an interest rate of 8.2% per annum (compounded annually), requiring a minimum annual deposit of ₹250 and a maximum capping of ₹1.5 Lakh per financial year.
