1. The Succession & Deceased Claim Bundle
When an account holder passes away without a joint survivor, these forms allow the bank to release the funds to the legitimate claimants.

Nomination Form DA-1:

Purpose: This is the most important form you can file while alive. It allows you to nominate an individual to receive the balance of your account in the event of your death.

Requirement: It requires the nominee’s details and a witness’s signature. If the nominee is a minor, you must also appoint a guardian to receive the funds on their behalf.

Claim Settlement Application Form:

Purpose: The primary application filed by the legal heirs or nominees to claim the money left in a deceased person’s savings or fixed deposit account.

Required Attachment: A certified copy of the Death Certificate is mandatory.

Specimen of Form to be Completed by Claimant(s):

Purpose: A detailed questionnaire for the claimants. It asks about the existence of a Will, the relationship with the deceased, and whether any other legal heirs (like siblings or children) exist who might have a claim.

Specimen of Indemnity Letter:

Purpose: A legal “guarantee” provided by the claimants to the bank. By signing this, the claimants agree to compensate the bank if someone else later comes forward with a legitimate legal claim to the same money.

Stamping: This letter must usually be executed on Non-Judicial Stamp Paper of a value prescribed by the State Government.

These documents cater to specific categories of account holders, such as Members of Parliament (MPs) and Central Government pensioners.

Declaration Form MPs – Declaration:

Purpose: A specific declaration used by Members of Parliament (MPs) regarding their banking and financial status. This is often linked to the disclosure of assets or the handling of their salary/pension accounts under specific government mandates.

Information of Pensioners Called by CPAO:

Purpose: The Central Pension Accounting Office (CPAO) manages pensions for Central Civil Pensioners (including former MPs and Judges).

Details: This document is used to update the pensioner’s data in the CPAO database. It typically includes the PPO (Pension Payment Order) Number, Aadhaar, and bank account details to ensure that the “Special Seal Authority” (the green signal for the bank to pay) remains valid.
