• Illiterate Customer Provisions: Account holders who cannot read or write must provide a thumb impression. This impression must be physically attested by two independent witnesses.
• Minor Accounts: If an account or locker is held solely by a minor, a lawfully entitled guardian must complete and sign the document. A dedicated table is provided to register guardian details for minor nominees.

• Supersession Rule: Signing this form automatically cancels and supersedes all prior nomination structures tied to the registered account or locker numbers.
• Digital Submission: The bank allows this form to be completed and submitted electronically if e-nomination facilities are active.
