1. Specific Use Case

2. “Former or Survivor” Definition

Under this specific mode of operation, the “Former” (the NRI) is the primary person authorized to operate the account.

The “Survivor” (the Resident Indian) typically gains operational rights only after the demise of the Former.
3. Required Information

Applicants must provide the names of all account holders and the specific NRO account number(s) affected by the change.

If the request follows a specific communication from the bank, the relevant letter number and date must be referenced.

The form requires the signature of both the 1st Applicant and the 2nd Applicant, along with the date and place of signing.
4. Verification and Receipt

The form includes a Customer Acknowledgement Copy that must be signed and stamped by an authorized official if submitted in person.

The bank maintains a “Branch Use” section to record the branch code, the date the receipt was acknowledged, and the date the action was finalized.
