• Aadhaar Consent: Voluntarily authorizes the bank to conduct UIDAI authentication and link the Aadhaar to all existing and future profiles/Customer Information Files (CIF).

• For DOB Updates: A self-attested copy of a valid age proof document is mandatory.
• For Biometric DBT Activation: Requires an e-KYC validation report alongside an attested physical copy of the Aadhaar card.
• Alternative OVDs: If biometric authentication fails due to injury, illness, old age, or worn fingerprints, acceptable Officially Valid Documents (OVDs) include Passport, Driving License, Voter ID, NREGA Job Card, or a Letter from the National Population Register.

• The bank official must certify whether biometric e-KYC was successful.
• A detachable Acknowledgement Slip at the bottom is signed and dated by the branch official to serve as the customer’s receipt.
