4. Welfare Services & Bank Linking (Fields 8 & 9)
- Consent Options: The applicant can explicitly check “Yes” or “No” to allow UIDAI to share data with state welfare delivery agencies.
- Bank/Post Linking: Offers checkboxes to either open a new bank/post office account linked to the Aadhaar or link an existing Bank of Maharashtra account (requiring Bank Name, Branch, IFSC, and Account Number). Note: This selection cannot be corrected post-enrolment.
5. Verification Modalities (Fields 10 & 11)
Applicants must choose exactly one verification stream:
- Document-Based: Requires valid physical proofs for Identity (POI), Address (POA), Date of Birth (DOB), and Relationship (POR).
- Introducer/Head of Family (HoF)-Based: Used only if the applicant completely lacks valid identity papers. It requires the Introducer’s or HoF’s Aadhaar number and signature to legally vouch for the applicant.
Final Execution Block
- Signatures: Requires the applicant’s signature/thumbprint and the formal stamp/signature of the verifying official.
- Agency Metadata: A bottom section reserved strictly for the Enrolment Agency (Micro Technologies India Ltd, Agency No. 1271) to stamp the exact date and timestamp of the transaction.
