Basic Submission Rule
- Target Audience: Strictly for Indian Bank customers only.
- Submission Process: Form must be completed with all required details and handed directly to the Branch Manager.
- Branch Routing: Directed to the specific home branch where the customer’s account is maintained and linked to the card.
1. Customer Information
- Customer’s full name.
- Account number.
- Debit Card or ATM Card number.
- Contact telephone or mobile number.
2. ATM Information
- ATM ID: To precisely identify the machine.
- Location: Required if the ATM ID is unavailable.
- Bank Name: The name of the bank operating the ATM (supports both Indian Bank and third-party ATMs).
3. Nature of the Complaint
- Cash Withdrawal Discrepancies: Tracks amount requested, amount actually disbursed, and amount debited from the account.
- Time Tracking: Requires the exact date (mm/dd/yy) and time of the failed transaction.
- Card Capture: Dedicated field to report if the ATM machine physically retained or swallowed the card.
- Other Issues: An open section for any alternative transaction complaints.
