📄 General Form Instructions

  • Language & Case: The entire application form must be filled out in English and in BLOCK letters using black ink (pp. 5, 7).
  • Dates: Every date entry across the form must strictly follow the DD-MM-YYYY format (p. 7).
  • Signatures: Must preferably be done in blue ink (p. 2). They can be in English, Hindi, or any official language listed in the 8th Schedule of the Indian Constitution (p. 2). Left Thumb Impressions (LTI) or signatures in other languages require official attestation by a Magistrate or Notary Public (p. 2).

🗂️ Mandatory Documentation & Proofs

All copies of submitted documents must be self-attested and accompanied by their originals for verification (p. 6):

  • PAN Card: A self-attested copy of the PAN card with a photograph is strictly mandatory for all applicants (p. 6).
  • Identity Proof (PoI): Unique Identification Number (UID/Aadhaar), Passport, Voter ID Card, or Driving License (pp. 5, 7). If a passport or driving license is given, providing the expiry date is mandatory (p. 9).
  • Permanent Address Proof (PoA): Ration Card, Passport, Driving License, Voter ID, or Bank Passbook/Statement (pp. 2, 5).
  • Correspondence Address Proof: Electricity Bill, Gas Bill, Telephone Bill (Landline only, not more than 3 months old), or a Lease/Sale Agreement (pp. 5-6).
  • Photographs: A passport-sized photograph of each account holder must be affixed and signed across (pp. 2, 5).

⚖️ Regulatory Declarations & Tax Compliance

  • FATCA / Tax Residency: Section 2 requires a tax residency self-certification (p. 3). Applicants must declare if they are tax residents outside of India and provide their Tax Identification Number (TIN) (pp. 3, 7).
  • Politically Exposed Persons (PEP): Applicants must explicitly declare if they are a PEP or related to one (pp. 5, 12).
  • Basic Services Demat Account (BSDA): Individuals can opt to avail of the BSDA facility to benefit from reduced SEBI-prescribed charge structures based on holding values (pp. 17, 19).
  • Nomination Details: Account holders must either formally declare their nominee(s) (up to 3 allowed) or explicitly state and sign that they do not wish to nominate anyone (pp. 12, 23).

💰 Key Schedule of Charges

  • Account Setup & Closure: Demat account opening and closure are entirely free (p. 21).
  • Annual Maintenance Charges (AMC):
    • Individuals: Free for the 1st year (p. 21). From the 2nd year onwards, it is ₹250 + GST (with a trading account) or ₹300 + GST (without a trading account) (p. 21).
    • Non-Individuals: Free for the 1st year (p. 21). From the 2nd year onwards, it costs ₹1,000 + GST for HUF and ₹1,500 + GST for Corporates (p. 21).

  • Transaction (Debit/Sell) Charges: For individuals/HUF, it is 0.04% of the transaction value (Minimum ₹30, Maximum ₹1,200 per ISIN debit instruction) (p. 21). A late submission incurs a penalty of ₹10 per ISIN (p. 21). Using the digital Speed-e/easiest portal reduces the rate to 0.02% (p. 21).
  • Other Essential Service Fees:
    • Dematerialisation: ₹5 per certificate (Minimum ₹100 per DRF) (p. 22).
    • Freezing/Unfreezing: ₹25 per transaction (p. 22).
    • Failed Debit Transaction: ₹50 per transaction (p. 22).
    • Delivery Instruction Slip (DIS): The first DIS booklet is free; subsequent booklets cost ₹100 each (p. 22).
