1. Personal & Account Registration
- PRAN Allotment: The form serves as an official request to open an NPS account and allocate a unique Permanent Retirement Account Number (PRAN).
- Name & Initials: The applicant’s full expanded name must exactly match their supporting identity proof document; initials are strictly prohibited.
- NRI Conditions: Non-Resident Indian subscribers must provide a valid Indian communication address and an active bank account located within India.
- Form Logistics: The application must be filled in black ink and block letters without overwriting, and include a non-stapled 3.5 cm × 2.5 cm color photograph.
2. Bank & Nomination Details
- Conditional Banking: Providing bank details is optional, but if any single detail (like account number or IFSC) is entered, all other bank fields automatically become mandatory.
- Nominee Split: A subscriber can nominate up to three individuals, and the designated percentage shares allocated to them must be whole integers that collectively sum to exactly 100%.
- Minor Safeguard: If any chosen nominee is a minor, registering the full legal details of a designated guardian is mandatory.
3. Pension Fund Manager (PFM) & Scheme Preferences
- Mandatory PFM Choice: Subscribers must actively choose exactly one Pension Fund Manager from the approved list, or the form will face summary rejection.
- Active Choice Parameters: Under Active Choice, subscribers can manually allocate money into asset classes E (Equity), C (Corporate Bonds), and G (Government Securities); however, Equity allocation is strictly capped at a maximum of 50%.
- Auto Choice Default: If the investment choice section is left entirely blank, the system automatically routes the subscriber’s funds into the default lifecycle-based Auto Choice framework.
4. Legal & KYC Declarations
- Legal Eligibility: The subscriber must declare that they are a citizen of India, of sound mind, not an undischarged insolvent, and do not hold a pre-existing NPS account.
- Anti-Money Laundering: Applicants must certify that all financial contributions are derived from legally declared and assessed sources of income.
- Physical Verification: Subscribers must present their original identity and address proofs alongside two self-attested photocopies at the POP-SP counter for real-time verification.
